Note: Ch. 12 describes the process for implementing an entire EHR into a practice; in this assignment, you will only work on 1 small database within the EHR, so you do not need to complete everything described in the chapter. System Life Cycle Framework

Planning Phase

Navigate to Healthy People 2030’s Browse Evidence-Based Resources page.

From under the Health Conditions heading, choose one of the health conditions to use as the subject for this assignment.

For the condition chosen, select one objective related to the condition. You will use that objective as the basis for your database (i.e., your database will collect the data necessary to measure whether the objective is met).

Determine the stakeholders for this objective.

Create your project scope statement using the objective you selected.

  • A scope statement outlines the entire project, including any deliverables and their features, as well as a list of stakeholders who will be affected. It will also include any major project objectives, deliverables, and goals to help measure success.
  • Determine your measurement for success. Define the specific conditions that will be considered successful.

Analysis Phase

Determination of Information Needs Conduct a needs assessment for the objective. This means determining what specific data must be collected to measure the objective as well as the standardized terminology as defined by CMS and ONC.

Develop an entity-relationship diagram showing the tables that are needed for your objective and how those tables relate to each other.

Develop a workflow and functional design document for your objective. See p 204 of Essentials of Nursing Informatics for a description of these. This will include the logical sequence for collecting the data on a workflow diagram (include who collects what data, the frequency, and how it should be collected), and what data is to be collected and how that flows to reports.

Design, Develop, and Customize Phase

Establish a communication plan to educate stakeholders on the objective. Consider Rogers’ Diffusion of Innovation to determine how to communicate with each group of stakeholders.

Identify barriers and risk factors that might disrupt the success of this project. Determine appropriate ways to eliminate or mitigate those factors and barriers.

Define how this information will be visualized and disseminated. If on a report, determine how it will be presented.

Implement, Evaluate, Support, and Maintain Phase

Enter information for a randomized population of 50 patients. (You can obtain information from any database that relates to your selected population. For example, use Healthy People 2030 to locate data for the appropriate population for your selected health condition. Other options can be found through healthdata.gov or data.world).

Evaluate the results of the project to determine if anything needs to be changed.

Produce the report on the objective that shows the results and compares those to the established benchmark.
