I think your fishbone looks good, I would just add lack of Telehealth services for the Equitable health Services line. You seem to hint at a limited technology options for connection and communication. Maybe you can through that in there and maybe shift some of the titles of the issues on your fishbone. Like for instance, Communication can be affected by lack of technology programs or Telehealth options such as apps, zoom meeting, eductions ect. so maybe creating a title where you merge technology with communication to make your fishbone more concise and bold. But this is just preference, I think its good but just these little adjustments in my option can make your fishbone better.

I think your diagram is really well put together. It is straight to the point and lays out the reasons for your issue. I do think if possible, you should elaborate a bit just give a little more detail so its very clear exactly what is going on. Overall the diagram is very good.
