As discussed in Chapter 8, treatment plans are what follow the assessment process and are created in collaboration with the client. There are many different templates for treatment plans, but some of the basic information gathered is outlined in your text as well as some additional examples in this week’s module which can be used for reference. As stated in the text, the treatment plan listed is only a small segment of a broader and more detailed plan as there may be additional items to consider, particularly if a mental health disorder is identified.

Read Case 8.2 in your text and answer the following questions below in detail. Cite in APA format where/when needed.

1) Why is consistent documentation of Anthony’s visit with the practitioner important? (minimum of 1-2 paragraphs) – 3 points

2) Create goals and a treatment plan for Anthony using the format from the text (I. Problems, II. Goals, and III. Interventions). Clearly support and cite your reasons for choosing the goals and treatment plan. – 10 points

3) Imagine what the next session with Anthony might entail, and write a progress note documenting that session that contains where the service occurred, length of contact, and a description of services and/or interventions provided. Do not use conjunctions in your notes as some agencies do not allow. (minimum of 1 -2 paragraphs) – 5 points

4) Provide feedback to another student that includes information such as: thoughts on their goals/treatment plan or notes, comparisons to what you wrote, support of their plans, analysis of their goals/treatment plan, would you do something different, and why? – 2 points
