You will be required to submit a monthly report and log to describe what you have been doing at your field placement and how your experiences meet the learning goals you identified in your Learning Contract. For example, if gaining group facilitation experience is one of your learning goals and you have the opportunity to run a group, you should provide a detailed description of your role and the progress being made toward your learning goal.
Each report should be 1-2 pages in length (not counting the hourly log) and written in APA format. At least 3 scholarly references are required in order to connect your experiences to theoretical or research-based information. For example, if you are using CBT as a counseling technique, you might make a connection to the theory behind this technique and how it has been effective with your population. In other words, make connections to the ‘why’ behind your techniques and activities. It is acceptable to write in the first person and use “I” when describing your experiences.
Each report will cover a 4-week time period and is due within 5 days of the end of that period. This is to allow you time to complete the written narrative and have your supervisor sign the log of hours. A 5 point per day deduction will be assessed to late reports.
