In short, you may utilize the causal analysis worksheet or full investigation report form to document all the tasks from the scenario. The scenario is meant to walk you through step by step analysis. Then completely answer the challenge questions.
The scenario begins with an overview and information obtained by you from an equipment operations expert. I then gets into immediate cause assessment. You begin here by identifying what substandard conditions you should look for. This starts at the regulations that are applicable because meeting regs is the minimum of safety. This machine is used in logging operations. Now we have to find out about its requirements and inspect for them. We use pictures of a different dozer to substitute for the actual machine, so dont let this confuse you. The pictures of the Case 850 provide a walk around view of you inspecting it.
The pictures of the Case machine used for substitute also show where the back up alarm indicator is located, it then shows the batteries and the connections for the back up alarm. This includes the solenoid on the transmission that is often disconnected in order to silence the alarm (this was just for your future reference/education) The pictures also show views from the cab seat. You should be able to quickly assess whether the machine met requirements. This Case substitute machine mimics actual conditions of the original machine.
