There must be a plan for every change in practice. You will create a strategic plan of the roll-out with consideration of other facility priorities at the time of the change. No change in practice is complete without evaluation. There may be the need for consideration of minor updates to the change for the facility or population affected by the change that will arise from the evaluation. The roll-out and evaluation must be something done by registered nurses, nurse practitioners, educators, or nurse administrators and/or over which they have decisional control. 

You may have discussed this project with your manager/director and sought some advice and directions regarding the feasibility of your implementation plan.

This is a plan to measure the actual plan, not to get “buy in”, permission, to teach anything etc. Let us assume that you are embarking on this intervention now. Let us assume that you have permission. You have implemented the plan, now how are you going to evaluate the plan?
Include how you will evaluate the proposed change in practice. (e.g. I plan to implement a new pain checklist, I should be evaluating if the unit has accessibility to the checklists, the issues staff are encountering (e.g. understanding the checklist etc.)
 

  • You have to decide how to do this before you implement the plan.  Every proposal will be different and the evaluation may differ from project to project.
