Current Medications

Heparin | 5,000 units | Subcutaneous every 12 hours 

D50W | 12.5 g every 15 minutes PRN for a blood glucose <70

  • Repeat until blood glucose is above 70 
  • Insulin detemir | 20 units | Subcutaneous | B.I.D.
  • Insulin lispro | Sliding scale | Subcutaneous ACHS (before and after meals)
  • Sliding Scale 

BG 170-190 = 1 unit

BG 191-200 = 2 units

BG 201-210 = 3 units

BG 211-215 = 4 units

BG 216-220 = 5 units

BG 221-230 = 6 units

  • BG 231-240 = 7 units
  • BG 241-250 = 8 units

BG >250 = Call MD
