Here is what I selected: 

Identity 1 – Gender 

Identity 2 – Ethnicity 

Health Risk 1 – Heart Disease in Men/Males

Health Risk 2 – Diabetes in Filipino Americans

attached will be A. the grading rubric and B. the PDF file of more clear and detailed instructions for this Personal Health and Identity Paper. I am also attaching all 5 sources that I’ve already found and got approval from the professor as well. That file is titled “Annotated Bibliography” 

quick correction for the works cited page – this is a comment that my professor shared: “the title of the journals should be capitalized (1st and 5th entries are incorrect). otherwise perfect!” you will see what she means when you look at the annotated bibliography PDF file.
