According to the World Health Organization, the number of people diagnosed with diabetes rose significantly from 108 million in 1884 to 422 million in 2014 (2021). The diagnosis of diabetes also comes with a higher risk of blindness, kidney failure, heart attacks, stroke, and limb amputation (WHO, 2018). In an effort to lower the prevalence of diabetes and complications that comes with the diagnosis, TriStar Health is proposing a diabetes education program that will launch in the middle Tennessee area. This program has three recommendations that will work to increase diabetes literacy and teach those living with the diagnosis the importance of a healthy and active lifestyle. The first recommendation for the education program will be to raise understanding of the diabetes diagnosis by providing a class led by a licensed diabetes educator. This class will provide information on how diabetes affects the body and how each of the associated numbers is important. Participants will be given surveys throughout the program to assess understanding and track health literacy over the course of the program. 

The second portion of the program will be helping the participants understand the importance of a healthy diet. A nutritionist will be utilized for this portion of the program helping individuals feel confident in making healthy decisions that will raise their quality of life through diet. The final portion of the program will be tracking physical activity over the duration of the program. Participants will be asked to track their physical activity as well as increase their activity over the course of the program. The individuals should see a direct correlation between their diet and physical activity and the associated diabetic numbers. These numbers will be tracked and progress will be discussed with the participants periodically throughout the program. The success of the program will be determined by the progress of the individuals as well as the retention rate throughout the duration of the program. Regular evaluations will be conducted throughout the program to analyze progress and success and changes will be implemented as needed to ensure the best possible outcome for all parties involved.
