Community Health Assessment and Improvement Plans are effective tools in practice. While focusing on different aspects of public health nursing, populations require different care priorities dependent on the perceived threat to overall health. Community Health Assessment allows professionals to take inventory of current resources available to the population and identify the existing gaps in care (Okura, 2019). During the initial stages, data is collected through; surveys, community focus groups, interviews with community leaders and organizations, and health-related studies. Improvement planning allows for the analysis of data, outlines health disparities, and develops long-term plans to address disparities (Okura, 2019). These tools further establish overall goals of interventions, establishing accountability of leadership. Implementing identified interventions requires the engagement of experts and leaders in the focused health field. Furthermore, implementation requires a multidisciplinary approach, including this process in the daily practice of a Public Health Nurse. This tool can also assist in the assessment stage of the process, ensuring that proposed interventions truly improve the community’s overall well-being. All tools used in public health each have strengths and weaknesses (Okura, 2019). However, community assessments and planning are imperative for all community health initiatives and practice changes (Okura, 2019).
