Instructions: The evaluation proposal uses scholarly literature to justify how you would design an evaluation.

Part A – Introduction and Statement Problem: The introduction should include a brief statement of the problem (a paragraph or 5-7 sentences) to be addressed by a social intervention (typically that your agency provides) and information concerning the prevalence (i.e., the number of people in a population who have the problem), and costs (social and financial) of the problem. This section should conclude with a discussion of why the problem is important to the field of social work.

Problem Statement and Purpose of your evaluation: Explain the problem, the population(s) affected by problem, and the intervention you are examining in the evaluation. Briefly discuss the significance of the problem and the intervention and their relevance to both social work practice and research. Briefly state how you will evaluate the impact of the intervention on the problem.

Part C – Program Description and Logic Model- Use a participatory process to engage key stakeholders in developing the logic model.

Briefly describe the program’s goals and objectives. Describe the nature of the clients served (e.g., race/ethnicity, age, geographic location), and any additional information you feel is relevant to explaining the program.

Logic Model: Present your program logic model in a diagram (Google logic model templates).

Indicate in your paper the specific area or areas on the logic model that will be examined in your evaluation.

**Key Notes for Evaluation Proposal**

The purpose of the evaluation is to evaluate if the First Offenders Program is effective in decreasing recidivism for its clients.

Agency: chosen agency for this project is Georgia Justice Project (GJP) specifically their First Offender Program.

Some of the most common issues people are faced within the program that should be addressed are ineffectiveness (some have stated their conviction was never successfully “expunged”) non-transparency (not all convictions are eligible for this program), and financial burdens (probation fees, court fees, etc).
