Patient’s Health History: Congestive Heart Failure. smoking since 13 years old, taking an 8 months break from it and drinking. The patient has pain in the lower back due to a car accident. Pain gets worse when bending over, and lifting up heavy things. The patient eats healthy and walks. the Patient’s perception of health is when someone can do everything for oneself.
