Hello everyone. I am Phyllis Caudill, and it is hard to believe I will actually complete my course of study in December! I have been fortunate to have great preceptors, both physicians and nurse practitioners who have shared their wisdom and skill throughout each clinical rotation. My confidence has increased with each rotation, and I am beginning to think of myself as their colleague. This is because I am no longer reticent about stating an opinion or asking a question regarding patient care. This is because my preceptors listen, encourage my research, and discuss each patient with me.

A study conducted by Schirle, et al. (2020) showed that barriers to an optimal practice environment included restrictions on practice, poor physician, and administrator relations, and misunderstanding of the APRN role. These barriers were associated with disappointment and increased intent to leave the job. The study identified that positive relationships between the physician and nurse practitioner and having autonomy and the opportunity for unconstrained practice facilitated an optimal practice environment. The study emphasizes the importance of physician and administration interactions, organizational-level guidelines, and colleagues’ discernment of the APRN role in furthering effective practice environments. It suggested that organizations should line up policy reform efforts through factors that further positive APRN practice environments to utilize the talents of the APRN efficiently and effectively in health care reform (Schirle, et al, 2020).
