The risk of black women dying from pregnancy-related health conditions including chronic diseases in the United States are increasing over the past two decades. These disparities have been a major concern for centuries in the United States. According to the Centers for Disease Control and Prevention (CDC), Black women are three to four times more likely to die from pregnancy-related causes than white women. This information is not new to public health, however, it continues to be a public health issue that has increased in its need for ensuring that black women are aware of the risk and how they can take action towards prevention. This would also align with the HEAR Her campaign that is currently being promoted by the Centers for Disease Control and Prevention (CDC).

Why the intervention will reduce the outcomes of premature births among black women

My capstone Proposal for the Peer 2 Peer Community Health Advocate program is an addition to the Community Health Worker programs that the Maternal Infant Health Network (MISN) has in place to support the maternal health of women, especially black and brown women in high-risk neighborhoods of the Hudson Valley area The program will focus on advocates located in the provider offices, who are trained and have lived experiences with chronic diseases and pregnancy and who will further support the recommendations of medical providers by providing education and experiences to women on how successfully manage chronic diseases during pregnancy and reducing the risk of preterm labor. This intervention is based on reinforcement and promoting positive patient outcomes when it comes to managing chronic disease. The fact remains that clinicians are unable to address the deeper complex, rooted social issues and often treat late manifestations of preventable disease (Kangovi et al., 2018).
